The seed bags we distribute have a label attached. This form is designed to be filled in on-line with drop-downs for easy input, printed at your site on a peel-n-stick label, and attached to the current label on the bag.

To prevent this label from lifting, please staple at least twice
even if it appears to be adhering ok.

Half-page labels may be purchased from Office Depot/Staples/etc and are Avery-6573 or go online for label number OL159WX from www.onlinelabels.com. The labels will be about 2" too large but may be cut or wrapped onto the current bag label. Please remember to staple at least twice.
