MTA Forms

Select the appropriate form to submit a request for MTA to accompany your order of certain varieties from Washington State Crop Improvement Foundation Seed Service (FSS). Because WSCIA works with both WSU and OSU, you may need to submit both forms if requesting germplasm from each breeding program. When you submit the form, your request is routed directly to the responsible technology transfer contact, who may contact you for more information and/or to provide an MTA for signature. Regarding WSU Varieties: This form must be used each time a non-licensee requests seed stocks of licensed varieties. Before the order for seed can be filled, the variety owner must notify FSS that the appropriate paperwork is in place to allow the material transfer. Regarding OSU Varieties: This form must be used each time seed of a licensed or public variety is requested for research purposes.
